What to Expect

Here’s our easy 5 step process to get you from today to your first appointment

We do our best to make getting started as easy and clear as possible. Please view each step below to know what to expect as you make your first appointment.

First, schedule your appointment by filling out this form or by calling our team at 877-339-2873.  Just call/text or use the web embedded calendar under your preferred provider.  You can also submit your information with the “check your coverage” button.

 

Once we receive your communication, our team will review your information and contact you to add your appointment to the provider’s calendar.

You will receive an email invitation to set up your Charm PHR (patient health record). 

 

This portal allows you to complete necessary paperwork, access appointment details, view/pay invoices and communicate with your provider.

After logging into the Charm PHR, you’ll find your intake paperwork under “Questionnaires.”  These questionnaires will also be emailed to you.  

 

Please complete these forms as soon as possible to ensure a smooth first visit. 

 

Please Note: Failure to complete all assigned questionnaires or the Headway setup at least 48 hours prior to your visit could lead to your provider cancelling your appointment.

If your provider uses Headway, you will receive a sign-up email from Headway.

 

This is used for billing and insurance purposes. Follow the instructions to verify your account.

Patients with EXCELLUS BCBS will not use HEADWAY in most circumstances. Other BCBS providers may be billed internally or with HEADWAY.

After all forms and assessments are completed, you’re all set for your first visit!

If you are scheduled for a virtual visit, just log into the Charm Health PHR to conduct the visit by “checking in” at the time of your appointment. 

You will also receive emails and text messages with the link to join the visit!  Any balances due will be paid that day with the credit card on file.

Tip for Virtual Visits:

Our virtual appointments are conducted through Zoom. To make your visit smooth and stress-free, please be prepared to join the Zoom meeting in one of two ways:

  • Using the Zoom App: If you already have Zoom installed on your phone, tablet, or computer, simply click the meeting link and the app will open automatically.

  • Using a Web Browser: If you don’t have the Zoom app, you can still join by clicking the meeting link and selecting the option to “Join from Your Browser.”

We recommend testing your device before your appointment to ensure your camera, microphone, and internet connection are working properly. This will help you get the most out of your visit!

2. Receive Your Patient Portal Invitation

  1. You will receive an email invitation to set up your Charm PHR (patient health record). 
  2. This portal allows you to complete necessary paperwork, access appointment details, view/pay invoices and communicate with your provider.

3. Complete Your Intake Paperwork

  1. After logging into the Charm PHR, you’ll find your intake paperwork under “Questionnaires.”  These questionnaires will also be emailed to you.

4. Set Up Your Headway Account (If applicable)

  1. If your provider uses Headway, you will receive a sign-up email from Headway.

5. Prepare for Your First Appointment

Longitudinally track clinical endpoints to aid in the monitoring and management of many clinical conditions and treatments e.g., measure the response to treatment like AD/HD medication, rehabilitation efforts, cognitive behavioral therapy – CBT, etc. and used to measure clinical outcomes
